Guide to writing a statement of informed consent
The statement of informed consent and the information sheet are normally separate documents. However, in some cases the researcher might wish to combine information and consent in a single document, for example if the project is straightforward and requires little explanation. In such cases it is important that the information and consent sections of the form are separated clearly.
The University of Tasmania’s Human Research Ethics Committee Social Sciences Consent Form could be helpful.
Information to be included in the statement of consent

A statement by the participant in the following terms:
- I have read and understood the information sheet for this research.
- The nature and possible effects of the research have been explained to me.
- I understand that the research involves [briefly describe what participants will be asked to do, mentioning any photographs, audio or video recording, and the approximate time commitment].
- I understand that [describe any risks or possible discomfort. If there are no foreseeable risks then state this].
- I understand that all research data will be treated as confidential.
- Any questions that I have asked have been answered to my satisfaction.
- I agree that research data gathered for the study may be published provided that I cannot be identified as a participant in the research.
- I agree to participate in this research and understand that I may withdraw at any time without any effect. [Note the term ‘without any effect’ should be put into context, e.g. for Anglicare clients it should be specified that withdrawal from the project will not affect their relationship with Anglicare services.]
